How to Pay
E-Transfer: Send it to info@citrusmedical.ca. Please note that this will be an automatic deposit to "1919145 Ontario Inc". Sometimes a sender's financial institution does not support the Auto deposit feature. If this is the case we ask that you please make the answer to the security question "doctor".
By Phone (Credit Card Only): Call us at 416-800-6500 and you can pay a fee by providing your credit card information.
In-Person: Pay with a debit card, credit card, or cash in the clinic. Please try to bring the exact amount of cash as change may not always be available.
Medical Records
I have a new family doctor, I want to transfer all my medical records...
Step 1: Provide signed consent
Option 1 - Fill out a request form at your new family doctor's office and have them fax it to Citrus at
416-342-1790 OR Option 2 - Fill out our Health Record Consent to Release Form with your new family doctor's contact information at the top. Submit this form to us using our Contact Form
Step 2: Fee for transfer
Once we have processed the request, we prepare the records and notify you of the exact fee via email. The standard charge is $30 for the first 20 pages + $0.25 per additional page.
Step 3: Payment
Once you receive an email about the fee, send an e-transfer to info@citrusmedical.ca.
Note: Please allow 5-7 business days to process your chart request. If your request is urgent, please let us know and we will try to accommodate in a timely manner.
I want a copy of ALL my medical records...
Step 1: Submit your request to Citrus
Submit a request through our Contact Form with the details of the patient who you are requesting medical records for.
Step 2: Fee for transfer
Once we receive authorization from the doctor, we will prepare the records and notify you of the exact fee via email. The standard charge is $30 for the first 20 pages + $0.25 per additional page.
Step 3: Payment
Once you receive an email about the fee, send an e-transfer to info@citrusmedical.ca.

I want a copy of my SINGLE test or imaging result...
Submit a request through our Contact Form with the details of the patient and the specific report you are requesting for (type of test and date of service).
Note: Our staff are not permitted to release any medical reports without authorization from a doctor. Once your request is received, it is sent to the relevant doctor to request permission for release.
If you have recently completed the test and you have not received any follow up from your doctor, the test result has not yet been reviewed. Please book an appointment with your doctor to discuss the results and request for a copy of the report.
Requests for images, disks, or films of previous x-rays, MRI, CT, or ultrasound tests should be directed to the imaging facility as these are not sent to your family physician.
I need a form completed by a doctor
Most forms are NOT covered by OHIP and a fee will be charged for its completion by a Citrus doctor. The exact fee will be provided upon completion as the fee is dependent on the complexity and requirements of the form. See here for approximated rates for common form requests.
When requesting to have a form completed by a Citrus doctor, complete the following to ensure your form can be returned to you in a timely manner:
Read the entire form and prepare answers to all the questions.
Fill out any information that is required by you (i.e. personal information & details, your signature, etc.)
Submit the document through our Contact Form at least 48 hours prior to your appointment OR bring the form with you to your in-person appointment
Please allow 3-7 business days for the form to be completed. Additional time may be required if lab results are needed. If there is an urgency to the form, let us know and we will try our best to accommodate.
